Step 1

Fill Out the Order

Download the Handpiece Repair Ticket.

If you don't have an Acrobat viewer, you may download a free copy of the Adobe Acrobat viewer directly from Adobe Systems.

 Complete the Handpiece Repair Ticket on-line or print and then complete.

Step 2

Prepare Shipping Label

 Print the Prepaid Mailing Label. Affix label to suit mailing box. If you need a box, please request the Handpiece Mailer Package

Step 4

 

 Complete a Postal Service Form 3811 if you would like a Return Receipt.
